This is the page where you can order Decals for Industrialized Buildings or Stickers for Manufactured Housing.

For Industrialized Building Decals, you will be required to submit the Registration Number and PIN of the licensed Industrialized Buildings firm. You will also need to provide the OSFM Project # for the module you need decals for, as well as the number of decals you want to request.

When this is submitted to OSFM, the program administrator will review the request and assign a dollar value per decal. When your request is approved, you will receive an email with an Invoice that needs to be paid. Once your Invoice is paid, the Decals will be issued and mailed to you.
